Fixed cheek bits, such as Full cheek, Eggbutt & Pelham should be fitted snug against the lips. Snug not tight! The lips shouldnt be sqashed in! A snug fit minimises the bit sliding from side to side which can cause pinching to the inside of the lips.

All loose ring bits should be fitted 1/4" to 1/2" bigger than your fixed cheek bit. This is to prevent the loose rings pinching the skin as they slide through the holes. Always check the lips to make sure no pinching has occured. Do not fit loose ring bits too big as this will allow the bit to slide too much which will cause pinching to the inside of the lips where you cant see.
